Our paid quote form is provided by Typeform. Please do not enter a Social Security number or payment details in the initial quote request. If you choose to apply later, an insurer or authorized service provider may request additional information under its own notices and procedures.
Website data and cookies
Our hosting and security providers may process technical information, such as an IP address, browser information, requested pages, and access times, to operate the site, prevent abuse, and diagnose errors. We use Vercel Web Analytics to understand visits to the paid quote page and confirmation page. We do not intentionally send form answers to this analytics service.
We use the Meta Pixel for advertising measurement on the paid quote funnel. It sends Meta a PageView when /fblp1 loads and, after the embedded Typeform reports a successful submission, a one-time standard Lead event on /fblp1/thank-you. These events include the relevant page URL and standard browser, device, and network information. The Lead event also has a unique event identifier for measurement and duplicate prevention. Our implementation does not send Typeform answers, contact details, date of birth, ZIP code, health information, or an advanced-matching payload to Meta. It disables automatic event collection, removes non-approved URL parameters before the landing-page Pixel loads, removes all query parameters before the Lead event, and does not count a direct visit or page refresh on the thank-you page as a Lead. Meta may use cookies or similar technology in connection with this measurement.
How we use information
We use your information to respond to your inquiry, discuss your insurance needs, confirm service availability, provide quote-related assistance, maintain records of your choices, protect the site, and comply with legal obligations. We use your phone number for SMS only if you affirmatively opt in.
We do not sell or rent information submitted through this quote form. A quote request does not subscribe you to third-party marketing.
SMS communications
If you affirmatively opt in, Loyalty Life Agency LLC may use the mobile number you provide to send automated messages about your life insurance quote request, customer care, scheduling, and reminders. Mobile phone numbers and SMS opt-in data and consent will not be sold, rented, or shared with third parties or affiliates for their marketing or promotional purposes. SMS opt-in consent applies only to Loyalty Life Agency LLC and is not transferred to another sender.
We may disclose mobile information to service providers that help deliver our messaging and customer-care services, when legally required, in a legitimate corporate transaction subject to the protections in this policy, or at your direction. None of these exceptions permits third-party marketing or the sale or transfer of SMS consent.
